General Duties Include:
This position works under the guidance of the Program Coordinator or assigned supervisor and plays an important role in supporting families during early childhood development.
In this role you will:
· Conduct regular home visits with eligible HANDS families
· Build strong, trusting relationships with parents
· Provide support and education related to child development, parenting skills, and overall family well-being
· Connect families with health services and community resources based on their needs
· Offer consistent support during pregnancy and until the child reaches two years of age
· Monitor the progress of the child, mother, and family, providing encouragement and guidance along the way
· Track appointments and follow up to help ensure families receive recommended services
· Complete periodic assessments to respond to families’ changing needs
· Maintain a strict confidentiality of all participant information
This position is ideal for someone who is caring, organized, and passionate about helping families thrive during the critical early years of a child’s life.
Education, Training, or Experience:
· Bachelor’s degree in social work, Nursing, Early Childhood Education or bachelor’s degree in a Social Work or Behavioral Science related field with one year’s
experience performing case management services may also be considered
· A master’s degree in human services may substitute for one year of experience
· An associate’s degree in nursing with licensure in Kentucky or a compact state and two years of public health experience will substitute for education.
Special Requirements:
· You must be 18 years of age
· You must have telephone accessibility
· Must have reliable transportation and maintain a valid driver’s license.
· Must pass a Criminal Background Check
On the job training needed within 6 months of hire to advance:
· HANDS home visitor training. (Must be completed before family contact can occur)
· HANDS wrap-around required training (Specific topical areas checklist)
· CPR Certification
· Working knowledge of geographic area and/or ability to locate participants and community resources with use of maps, contacts and direction provided
Compensation and hours:
Salary negotiable with experience. Full-time 37.5 hours per week. This position does require some evening work.
